Abstract
In the United States, pharmacists are generally considered the most accessible healthcare professional and can play a vital role in managing chronic disease states, such as high blood pressure. High blood pressure, otherwise known as hypertension, is considered the most prevalent modifiable risk factor for cardiovascular disease per the American College of Cardiology/American Heart Association (AHA) Joint Committee and affects about 46.7% of adults above the age of 18 in the United States as of 2025. High blood pressure can be managed with adjustments to lifestyle and use of maintenance medications. The AHA also recommends the use of at-home blood pressure monitoring to track trends in blood pressure values, identify complications, understand medication adherence, and guide evidence-based approaches to therapy. This capstone enhancement focuses on implementing a remote monitoring program for ONU Healthwise Pharmacy patients with a history of uncontrolled blood pressure who are taking at least one blood pressure medication. Primary goals of this program include bringing patients’ blood pressure within goal range, optimizing medication regimens, and improving medication adherence. Secondary outcomes are quality improvement and a billable clinical service description.
