Abstract
Pharmacies are essential for the reception and filling of prescriptions, but they can also be vital for the monitoring and managing of different patient conditions and medication regimens. Patients, especially those who are older or live with chronic conditions, can be prescribed many different medications and therapies. Inversely, they might not have the medications and therapies needed to address their conditions. ONU Healthwise Pharmacy and the pharmacy call center utilize a platform called OutcomesMTM as a way of providing Medication Therapy Management (MTM) services to help patients control their chronic conditions and manage their health. These services, delivered through phone calls, include adherence checks, immunization recommendations, refill reminders, Comprehensive Medication Reviews (CMRS), and more. Each service benefits the patient while also providing monetary reimbursement to the pharmacy. This project will collect data on the effectiveness and profitability of each service utilized and provide the analyzed results to the pharmacy and call center. Additionally, it will focus on how to best utilize the services offered by OutcomesMTM to provide better health outcomes for the patient and a better financial outcome for the pharmacy. With specific focus on each service, the project will allow the pharmacy and call center to look at potential areas of improvement that would lead to an improvement in the overall health of patients. Ultimately, these findings will result in better strategies to best utilize OutcomesMTM for the benefit of the patient and pharmacy.
