Abstract
Chronic diseases require continuous management to decrease complications and improve quality of life, innovative interventions are needed to enhance patient outcomes. The telehealth approach offers a way to increase access to care and enhance regimen adherence, while also reducing costs. This study examines the effectiveness of telehealth interventions compared to traditional in-person care for managing chronic diseases. Using a convenience sampling method, participants will be recruited based on accessibility and willingness to participate. Key outcomes assessed included symptom control, medication adherence, and hospital readmissions. Telehealth can provide effective, patient-centered care and improve disease management and overall quality of life. Additionally, telehealth interventions offer convenience for patients with mobility limitations and those in rural areas, potentially reducing healthcare disparities. While some challenges remain, such as technology access and digital literacy, the results highlight the potential of telehealth to enhance chronic disease management.
